This is to inform you that on (Date), at approximately (time), Dr. (name) left a physically disabled patient, (name) unattended in the bathroom for (time duration. Despite repeated calls for help, which were brought to the attention of the concerned doctor by two other patients, (names of patients), the Dr.--- refused to leave the desk where he was writing on charts. The patient, Mr.____ finally attempted to leave the bathroom without assistance, which led to a fall resulting in a fractured right hip.

Kindly look into the matter and do the needful for the patients since it does not reflect nicely on your hospital.
